Logging Incident & Injury Reports in the Mobile App

The HydroApps mobile app makes it easy for staff to document incident and injury reports directly from their phone or tablet. Staff can quickly complete forms, document injury locations, validate required information, and submit reports in real time without needing to return to a desktop computer.



Starting an Incident or Injury Report

To begin a report:

  1. Tap the Quick Input (+) button at the bottom of the screen

  2. Select Incident Injury Report


The mobile app uses the standard HydroApps incident and injury form. If your organization has customized that form, your custom fields and settings will also appear in the mobile experience.



Entering Report Information

Staff can quickly complete all required report details, including:

  • Date and time of the incident

  • Individual involved

  • Incident details

  • Witness information

  • Care provided

  • Additional operational notes


Simply tap into each field and complete the necessary information as prompted.



Injury Body Selector

One of the key mobile app features is the built-in body selector tool.


When documenting injuries, staff can visually indicate where an injury occurred by selecting the affected area directly on the body diagram. HydroApps automatically records the selected injury locations within the report documentation.


This helps improve accuracy and consistency in injury reporting.



Form Validation & Required Fields

If your organization has required questions or validation rules configured, HydroApps will automatically enforce them before submission.


For example, if a required field such as refusal of care information is missing, the app will:

  • Prevent submission

  • Highlight the missing information

  • Navigate staff back to the required question


This helps ensure reports are fully completed before finalization.



Finalizing the Report

Once all required fields are completed:

  1. Review the report information

  2. Tap Submit

  3. Confirm finalization


The report is then officially logged and stored in HydroApps.

If form submission notifications are enabled, designated staff may also receive immediate notifications when a report is submitted.



Viewing In-Progress Reports

To access reports that are still being completed:

  1. Open the Navigation Menu in the bottom left corner

  2. Select Forms

  3. Choose the Incident Injury Report form


Be sure to verify the correct facility is selected using the facility selector so you are viewing the appropriate reports.


From there, staff can review and continue any in-progress submissions as needed.
